Usually some kind of undetected or undiagnosed heart condition.
Years ago I had a guy in my class drop dead at 17 from a "heart attack". Was a slightly enlarged heart and resulting complications.

Over a decade ago a friend died of a heart attack at 19 - he had a known heart condition but had been fine for several years. He was a nice guy that I can't recall ever being mean to another living person or creature - don't expect fairness in life!

Clarify: do you mean "replacing the actor playing the main character", "changing the show's focus from one character to another", or "changing the personality of the show's main character"?

Replacing the lead character, and actor. i.e. Dr Who. (As opposed to simply replacing the actor, but trying to keep the same character - that's never worked.)

Although "changing the show's focus from one character to another" might be interesting also, ie X-Files, Stargate.

Edit: Serial movies don't count (Batman etc), I'm talking TV series.
