Those things aren't directly caused by trans-inclusive legislation. Those institutions/organisations make decisions on how they run and structure themselves.
The prison example was clearly a case of
mismanaging a prisoner with a history of sexual violence.
With the MMA athlete there's no
evidence[that the injury had anything to do with her being transgender
And there's also no evidence that letting transgender people use public facilities increases safety risks.
